BIO-IDENTICAL HORMONE REPLACEMENT/ANTI-AGING PROGRAM

 

           1.  Initial comprehensive assessment - $425.00  

            Patient must fill out on-line questionnaire and fax to Lite Touch

●      Labs will be drawn based upon the history prior to initial appointment- will need insurance card (patient comes to Lite Touch for labs) – must be fasting

●      Special testing may be needed (mammogram, bone density, etc.)

●      Usually a 1 hour appointment after labs back

●      Medications/supplements ordered

        2.    Second Follow-up in 3 months - $275

●      1/2 hour to 1 hour

●    Follow up labs drawn before visit, prescriptions will be renewed or changed based upon lab work and symptoms

3.  Bhrt  follow-up in 3 or 6 months - $125

●      Depends on previous appointment - if no changes were made to medications -  then will be a 6 month appointment (skip to next line)

4.  bhrt  follow-up in 6 months - $125

●      Labs and medication refills

5.  Bhrt  yearly follow-up - $275

●      Includes medication/supplement orders

●      Laboratory work
 

Total costs for year: $1250-$1375 (without lab, medication or product costs)

(Includes Releana Weightloss program)

(No BHRT)

●      Medications/supplements ordered

●      Weight loss program started

      2. Follow-up visits (monthly for Releana for 3 months) -   

after labs and tests completed $250.00/month = $750

●      ½ hour to 1 hour

●      Medication and supplement adjustments

●      Price includes cost of Releana and visit

3. Yearly Follow-up - $125

●      includes medication/supplement orders

●      laboratory work

Total Costs for Year: $1300 (without lab or medication)
